device_create() issues a debug backtrace to the kernel log upon duplicate device name. Use a simple hash to detect this upfront in the element factory, returning gracefully with -EEXIST instead of having the kernel freak out. Signed-off-by:

EVL threads start running immediately after creation, there is no need for an intermediate stopped state. Let's open code the few bits required to set up the scheduling state of an emerging user/kernel thread instead, and get rid of this Cobalt legacy. Signed-off-by:

Significant optimizations can happen only when acquire/release ops involve single increment/decrement. Although we loose the ability to implement read/write locking, we can still do that using a monitor protecting an access map. Detect invalid requests such as recursive locking and unlocking of free mutex early. This is prep work before we can drop the nklock entirely, which is recursive by design and would somewhat prevent a kernel lockup upon invalid request. Once we switch to using per-wchan hard spinlocks, we don't want to risk recursive (spin)locking attempts when traversing a PI chain. Signed-off-by:
